Community Rehabilitation
WHAT WE DO
Community Rehabilitation provides the following services:
- Life and Work Skills in a structured environment; essential workplace skills are emphasized, such as communication, problem-solving, teamwork, adaptability, and work ethic
- Job training in assembly, labeling, packaging, sorting, sealing, light manufacturing, machining and more
- Learn and Earn model (paid) adult day training (ADT) model with a person-centered approach for success
- Individualized Plans– each CRS client has a personalized plan with goals for independence, social skills, and work readiness
- Specialized groups (women’s and men’s groups, arts, music, health/wellness, and more) to empower clients to thrive in the community
- Community Activities to promote integration and socialization

ELIGIBILITY & QUALIFICATIONS
Eligible candidates for Community Rehabilitation and Life Transitions generally includes adults with intellectual or developmental disabilities, who are also receiving the Michele P or SCL Medicaid waivers in Kentucky. Other funding or referral sources may also apply. CRS/Life Transitions clients reside in Boone, Campbell, Caroll, Gallatin, Grant, Kenton, Owen, and Pendleton counties of Northern Kentucky.
Life Transitions
WHAT WE DO
Life Transitions (a separate program operated by Community Rehabilitation) assists individuals with more significant disabilities, or other challenges, by providing social, interpersonal, and recreational opportunities in a comfortable and relaxed environment. Services emphasize stability and daily life functioning through the provision of highly interactive activities to assist with maintaining cognitive, physical, social, and daily living skills. With a higher staff-to-client ratio, Life Transitions clients receive intensive supports and personalized care.

Age range of admissions – Program is primarily structured to serve and support aging adults, but is open for younger individuals based on their needs. Individuals with intellectual or developmental disabilities. Serving residents in Boone, Campbell, Kenton, Grant, Gallatin, Pendleton, Bracken, and Owen Counties.
